Minoxidil: how it works, what forms it comes in, and what its side effects are.

Minoxidil was originally developed for severe, hard-to-treat high blood pressure as a potent vasodilator drug. StatPearls A side effect showed up during use: some patients grew denser, stronger hair. This observation later led to the scalp-applied product used for hair loss. Suchonwanit 2019 This is a medication, not a cosmetic.

Minoxidil is meant for patterned (androgenetic) hair loss; for more on the pattern and other options: male and female hair loss. Sudden, diffuse shedding is a different situation: there, you look for the trigger, and minoxidil's effect isn't proven. Read about it here.

The exact mechanism is still not fully understood. Minoxidil has a potassium-channel-opening and vasodilating effect, meaning it supports local blood flow. StatPearls On hair, the effect is exerted by minoxidil sulfate, the active form produced in the scalp by an enzyme (sulfotransferase). Suchonwanit 2019

The key point is in the hair's life cycle: minoxidil appears to lengthen the anagen, meaning the growth phase. Suchonwanit 2019 This keeps the hair in the growing state longer, and over time the hair can look denser and stronger. This effect was shown across groups of hair follicles in clinical trials.

Every hair follicle goes through this cycle: it grows in the anagen phase, for years; it retracts in the catagen phase; it rests in the telogen phase, then sheds, and the follicle restarts. Minoxidil intervenes here by making the anagen, the growth stage, longer. The exact molecular background is still an open question in research.

Scalp-applied minoxidil is available in both solution and foam form, in lower and higher concentrations. The main difference is in the carrier:

• Solution: contains alcohol and propylene glycol. Propylene glycol helps the active ingredient penetrate, but for many people this is exactly what irritates the scalp (itching, flaking, sometimes allergic contact dermatitis). Suchonwanit 2019
• Foam: made without propylene glycol, so it's typically gentler, absorbs quickly, and is less sticky. If the solution irritates you, foam is often the better choice.

Minoxidil is a real medication, with real side effects. The most common ones:

• Increased shedding at the start of treatment ("shedding"): minoxidil shortens the resting phase, and this switch can come with extra shedding. StatPearls It's a scary experience, but the phenomenon is a known part of treatment. If the shedding stays heavy, is patchy, or comes with a burning scalp, it's worth getting checked by a doctor.
• Scalp irritation: itching, flaking. Often caused by the carrier: the propylene glycol in the solution irritates many people's scalps, and can also trigger allergic contact dermatitis. Suchonwanit 2019 In that case, the propylene-glycol-free foam is typically gentler; if the foam irritates too, that may point to the active ingredient itself.
• Unwanted facial and body hair: the hair-growth effect doesn't stop at the scalp. The chance of it rises with higher concentration, and it's more common in women; according to the review, it typically recedes on its own within months after stopping, first on the face and arms, later on the legs. Suchonwanit 2019 It helps to apply it only to the scalp, wash your hands afterward, and let it dry before you lie your head on a pillow.
• Systemic effects: the major systemic side effects (fluid retention, weight gain, blood pressure effects) belong to the oral form; for the topical form, the review found no systemic side effects at twice-daily use, and in the arms of one study in women a small proportion reported heart palpitations. Suchonwanit 2019 If you experience heart palpitations, swelling, or dizziness, stop and see a doctor. If you have existing heart or vascular disease, talk to a doctor before you even start.

And the thing many people fear, that actually belongs to a different drug: stories about sexual side effects are typically about finasteride. Minoxidil works through a different pathway: a potassium-channel-opening vasodilator StatPearls, not a 5-alpha-reductase inhibitor, so the hormonal side-effect profile described for finasteride doesn't carry over to it. Minoxidil's effect is maintenance-dependent. As long as you use it, your hair stays on the growth side; but the gain depends on regular use.

Missing a single dose isn't a tragedy. Whether you keep your hair isn't decided day by day, so if you miss a day now and then, that alone doesn't upset anything. Just continue as usual, there's no point making up for it with a double dose.

Stopping long-term is a different question. Long-term use is needed for the achieved state to be maintained, because the effect fades once you stop. StatPearls The hair gained typically sheds back gradually over the following months to where your hair would be without treatment. Suchonwanit 2019 That's why it's worth thinking through at the start whether you're committing to the long term. If you want to stop, you can do so at any time, just expect the gradual reversal.

For hair, the strongest data is on the combination of microneedling and minoxidil. In an evaluator-blinded study, the group that also received microneedling grew meaningfully more new hair than those using minoxidil alone. Dhurat 2013

This study was done with a 1.5 mm clinical roller, one treatment a week, under controlled clinical conditions, over 12 weeks. Dhurat 2013 The Dermastamp is a fixed 1 mm, at-home device, and wasn't part of this study. The cited evidence is about the mechanism of microneedling: the needle stimulates the environment around the follicle.

The active ingredient penetrates more easily through fresh microchannels, so we don't apply minoxidil right away to freshly punctured, sensitive skin. Cureus 2025 The studied protocol skipped application on the day of puncturing, and resumed minoxidil only 24 hours after treatment. Dhurat 2013

In our hair-loss community (r/hajnovesztes), more than a third of posts are about sourcing, and I know many people order from foreign web shops. There's also a misconception: that only 2% minoxidil is available in Hungary, and you have to go abroad for the 5%. This isn't true. The medicines authority's database (formerly OGYÉI, now NNGYK) lists several 5% solutions marketed under Hungarian authorization, with a Hungarian patient information leaflet, all of them without a prescription NNGYK medicines database.

Here's how to check any web shop before you pay. In Hungary, only a pharmacy holding an operating license may sell medication online OGYÉI: buying medication online. On the pharmacy's website, check whether the common EU logo is there, whether you can click it, and whether it leads to the authority's list (a logo inserted as an image proves nothing). For a Hungarian site, the official pharmacy finder tells you whether a registered pharmacy really stands behind the web shop.

And what's almost never mentioned: the same rule also requires that the product comply with the national law of the member state of destination as well. In other words, a foreign web shop isn't lawful toward Hungary even if it's an existing, operating company in its own country 2011/62/EU.

Home delivery for medication doesn't work in the usual way. Under the regulatory rule, medication ordered online "can only be picked up in person at the pharmacy or delivered to the home by the pharmacy's professional staff" OGYÉI. So it can't come by a regular courier or a parcel locker, and many web shops only offer pharmacy pickup to begin with, which is also typical of the shops in the price table below. There's a pharmacy chain that also delivers to the home with its own professional staff, for a surcharge. When ordering, plan on a pharmacy stop, or choose a shop that has a partner pharmacy near you.

Check this on the box: whether it has an approved patient information leaflet in Hungarian · whether it has a registration number starting with OGYI-T- · whether the batch number and expiry date are legible. For reporting a suspected counterfeit medicine, the authority gives the address hamisgyogyszer@nngyk.gov.hu NNGYK.

How much does it cost? Specific prices, with stores. The same box goes for a noticeably different price from store to store, so it's worth looking at the price per milliliter. For Revicapil's 100 ml, 5% solution, at the time this article was updated (August 2026) I found these prices:

It was more expensive at BENU, but in exchange you can pick it up at more than 200 of their pharmacies, so pickup is almost certainly convenient. At the time of updating, the same box was several thousand forints cheaper elsewhere. Prices change; you can check the current state all in one place on PatikaRadar. The daily dose is given on the patient information leaflet; from that you can calculate how long one bottle will last.

And why isn't it at DM or Rossmann? In Hungary, a store (drugstore, gas station) may only sell the non-prescription medications that the authority has specifically classified for that purpose; among the conditions is that the medication's "hazard (side effect, interaction) is not significant even in the case of a considerably greater overdose" OGYÉI. On the data sheets, minoxidil products are listed with plain "non-prescription" status, without the outside-pharmacy (GYK) classification, so you can't get it at a drugstore, only at a pharmacy NNGYK medicines database.

And the reverse is a red flag: counterfeit experiences in the community are typically tied to conspicuously cheap marketplace listings. If somewhere offers "the same thing" for a fraction of the pharmacy price, the question already is what's actually in the bottle.

The bleach test: what it shows, and what it doesn't. There's a home test that lives on in the community: you mix a little solution with a little household bleach, and if the mixture turns a yellowish-orange color, it contains minoxidil. The logic is that bleach is a strong oxidizer, and according to reports it causes a color change with minoxidil. I found no published, controlled validation of it, so this is a home signal, not a lab measurement.

What a positive result can prove: that the liquid contains an active ingredient that reacts with bleach. What it doesn't tell you: how much is in it, and what else is alongside it. Those are exactly the two questions with a counterfeit. On our hair-loss forum, moreover, experience runs both ways: one person had one bottle change color and another not, with the same label, and there's also debate about how much the genuine product changes color. A negative result is therefore a strong red flag, and a positive one is a reassuring sign, but not proof.

If you try it: work with a few drops, in a well-ventilated space, and don't mix the bleach with another cleaning product, since it's a corrosive substance, and mixed with acid it can release chlorine gas. Pour out the tested amount afterward.

On forums it's called minoxidil tablets or "loose minoxidil." Oral minoxidil has no marketing authorization in Hungary; the products in the Hungarian registry are all topical solutions.

There is, however, an authority-designated route that almost nobody knows about. In November 2025, the NNGYK issued an off-label prescribing recommendation: according to it, minoxidil can be prescribed for androgenetic alopecia "by dermatology specialists under a simplified procedure," and the product is an individual import, which "depends on current availability" NNGYK, 2025.11.12. This route is initiated by the doctor, not the patient; the statutory deadline is 8 working days Government Decree 448/2017.. This doesn't mean it's freely accessible: availability varies, and the first step is an appointment with a dermatologist.

And one thing before anyone thinks of it as a "stronger Regaine": oral minoxidil was originally a blood-pressure-lowering drug, and it's used for hair loss outside its own indication FDA, Loniten. It's no accident that a specialist is the gatekeeper: someone has to weigh and monitor its cardiovascular effects.

On forums it's seen as the "more serious version," the one people switch to when the solution isn't enough. The trials say something different.

In the largest, double-blind trial, one tablet a day went up against the twice-daily 5% solution, over 24 weeks, and according to the closing statement, oral minoxidil "did not demonstrate superiority" JAMA Dermatology, 2024. In an analysis pooling four trials and 279 participants (which includes the trial above), there was likewise no measurable difference in hair density and hair-shaft thickness; there was one exception, unwanted hair growth was twice as common with the oral form Int J Dermatol, 2025. The limitation: "we found no difference" is not the same as "proven equivalent." What can be said is that the big advantage mentioned on forums didn't show up in these trials.

According to a pooled analysis of six trials, every +1 mg daily dose increase was associated with an average of +47.1 more hairs per square centimeter over half a year, but the same increase also came with a +17.9% unwanted-hair-growth risk and a +4.8% circulatory side-effect risk Skin Appendage Disord, 2022. More hair and more side effects are two sides of the same dose, you can't ask for just one. The limitation: this analysis compares data across six trials, so it shows an association, not proof of causation. Dosing is the prescribing doctor's job, who knows your blood pressure, your heart, and what you can tolerate.

Two things not to do. Don't drink the topical solution: the 5% solution contains 50 mg of minoxidil per milliliter, many times the tablet dose used for hair loss, and in 2024 the European medicines authority made it mandatory to add "Do not ingest." to the box, because of the data available on accidental ingestion EMA/CMDh, 2024. And don't order from an unlabeled source: by definition there's no manufacturer quality assurance and no regulatory oversight there, so you don't know how much active ingredient is in it or what else is in it.

Finally, something that goes against our own interest. This site is run by a company that sells a microneedling device, so you might expect me to tie the two together: there's no study on the combined effect of oral minoxidil and microneedling. We searched two major scientific databases and found not a single one that measured the two together search details at the source. And the existing data shouldn't be carried over to the tablet: the most-cited microneedling study was done with topical minoxidil, where needling may help, in part, precisely by opening a path for the active ingredient applied to the surface. With a tablet, the active ingredient arrives from the bloodstream, so this explanation loses its meaning.
